What Information Does This Site Collect?

We collect only the information you choose to share. Specific items of information you may choose to provide are:

Account Information

In order to create an account, you must provide a valid email address and a password. We encourage you to use an email address that does not contain personal information such as your name or the name of your internet service provider (ISP). There are many sources for free web-based email.

Demographic information

The following information can help us tailor the questionnaire to your specific situation:

  • Approximate date of birth (year and month)
  • Gender
  • The first 3 digits of your ZIP code (for rough geographical location)

Questionnaire Responses

You select the set of symptoms that apply to your situation. For each one, you rate the frequency or intensity on a three-point scale (low, medium, high). Providing this information allows the site to create personalized reports that summarize your health profile.

Analytics

Google Analytics is used to collect anonymous information about visitors to the site.

How is the Information Stored and Used?

Account Information

The email address you provide for login purposes is never stored in plain text. We only retain an encrypted one-way hash of the email in order to verify your identity when you return to the site. The account profile you create can be accessed by means of the email address and password you provided when you created the account. There is no practical means for anyone to determine that email address from the data that we store if they do not already know it.

The primary use for your email address is to authenticate you when you login to the site. It can also be used for password recovery if you remember the email you used to register, but have forgotten your password. If you forget the email address that is associated with your account, you will not be able to sign in.

Demographic Information

Your answers to the demographic portions of the questionnaire are stored in a database, and used by the site to determine appropriate questions to ask you. For example, some questions are gender or age-specific.

Questionnaire Responses

Your answers to the questionnaire are stored in a database, and can be referenced by you and your health care provider. The unique access code that you used to set up an account is the link between you and your provider. Since our system stores all data anonymously, that link is the only way for your health provider to know which data belongs to which individual.

Analytics

Statistics are aggregated anonymously across broad categories of site visitors. Examples of how this information would be used are: determining which days and times of day experience peak traffic loads, determining approximate geographic locations for site visitors, and the range of browsers and computer systems used to access the site. This information can help ensure the site is tested to operate well under the variety of configurations its visitors actually use, and to ensure any maintenance windows are scheduled at times of low demand.

Removal of Information

You can add, change or delete your demographic and questionnaire responses at any time. In addition, your health provider can delete your account information upon request. If you prefer, you can use the Contact Us page on this site to request that your account be deleted
